Uses skilled interventions and evidence-based practice to collaborate with patients, physicians, and multidisciplinary treatment team to achieve quality outcomes.
Performs utilization review and assists with levels of care classification.
Assists physicians with assigning appropriate level of care placement Facilitates discharge planning.
Provides necessary education and resources to meet the discharge needs of individual patients and families.
Collaborates with other disciplines to provide patient care across the continuum of services inside and outside the facility.
Documents patient care and responses to care per standards Identifies and follows up on potential quality, risk and service utilization concerns Other duties as assigned.
Maintain BCLS certification KNOWLEDGE, SKILLS & ABILITIES Self- motivated, ability to work independently and as a member of a team Strong relationship-building and communication skills (verbal and written) Ability to assess, plan, implement and evaluate age appropriate nursing care per ORCH policy/procedure and meet regulatory expectations from the Joint Commission and Center for Medicare/Medicaid.
Working knowledge of Medicare Managed Care, inpatient, outpatient, home health care and continuum care Knowledge of community services, resources and programs preferred Minimum Qualifications: EDUCATION Graduate of an accredited program of registered nursing EXPERIENCE 3-5 years of hospital based experience in the delivery of Acute care as a Registered Nurse.
